At the polling station:
- You will be asked your name.
- Your address
- For photo ID
- If you bring a companion to help, they will need to complete a form.
How people can help you:
- Explain the voting options to me.
- Come into the polling booth with me.
- Mark the ballot paper with my vote if I allow them to do so.
What people cannot do for you:
- Decide for me.
- Mark the ballot paper against my wishes.
- Stop me from voting.
Privacy statement
By filling out this form, you consent to us holding and sharing your personal information with election and polling station staff. The information you have given us will be used to provide you with support when voting in a polling station.
We do not share this information with other organisations or individuals.
Your information is securely stored on Warwick District Council's servers, however only the Electoral Services Team will have access to your information.
The Electoral Registration Officer is obliged to process your personal data in relation to preparing for and conducting elections. Your details will be kept and updated in accordance with our legal obligations and in line with statutory retention periods. We will keep your information to provide you with this service. Periodically, we will contact you to ensure that our records are complete and accurate and that we have your continued consent to hold your details. We will then dispose your information by deleting any online forms you have completed securely and confidentially, in line with our legal obligations and statutory responsibilities.
