Homeless Street Outreach Physical Health Nursing service

Street Outreach Nurses based in the local community supporting those experiencing homelessness, working with partnership with the local authority housing team and fellow commissioned homelessness services.

The team are dedicated to providing health care to the local homeless population, aiming to respond quickly to emerging health needs and engaging individuals with wider health services. Referrals can be made for any individual  resenting as homeless and in need of healthcare, including supporting hospital discharge to reduce the likelihood of readmission.

Housing Hospital Liaison Service

Working across hospital and community health settings to deliver proactive housing/ home interventions to achieve prompt discharge, reduce length of stay and help prevent delays in being discharged from hospital and prevent
unnecessary admission or re-admissions to hospital.

Housing Related Support (HRS) Services

Housing related support services support people with vulnerabilities and additional needs to enable them to attain the skills required to be able to gain and/or maintain a tenancy and live independently in the community and help prevent homelessness.

These services are offered either through accommodation-based support, where temporary housing and support are delivered together, or through floating support for those who need support to manage and maintain their accommodation if it is at risk.

Working with people, the support worker develops a personalised support plan to help improve people’s ability to maintain their housing and help them be independent after the support ends.

  • budget effectively
  • understand their tenancies
  • participate in training/education and employment

There are 3 Floating Housing Related Support Services.

Together deliver a countywide Disability Floating Support Service for young people and adults 16+ years (inclusive of those with enduring mental health problems):

Change Grow Live Warwickshire (CGL)

CGL is a free, confidential drug and alcohol service commissioned by Warwickshire County Council to provide drug and alcohol treatment for adults aged 18+, open to anybody experiencing difficulties with drugs or alcohol. We offer a range
of services that are tailored to the individuals’ goals, these include:

  • Advice and Support for individuals and professionals.
  • An experienced team of health and care staff; nurses, and doctors that offer comprehensive health screening, BBV testing, immunisations, substitute prescribing as well as home / ambulatory and access to inpatient detox’s.
  • Psychosocial Interventions: both one to one and group work.
  • Links to recovery coaches, community fellowships and support from people with lived experience.
  • Employments and Training Support.
  • Harm Reduction, including needle exchange and provision of Naloxone.
  • Support Group for Family Members (18+).
  • Recovery Support: offering a range of groups, including relapse prevention, wellbeing.
  • Free training for professionals.
  • Access to detoxification and rehabilitation.

Warwickshire Family Information Service

Warwickshire Family Information Service provide advice and one-to-one support for families with children and young people aged 0-25 across Warwickshire.

Citizen’s Advice Support

Citizen’s Advice can support with homelessness, or if anyone is likely to become homeless. In addition, Citizen Advice maybe able to help with referrals and emergency housing providers.

Compass Children and Young People’s Drug and Alcohol Service Warwickshire

Our Service can help with offering confidential 1:1 support and specialist advice for children and young people up to the age of 25, family work, hidden harm, group work and transitional care.

Preventing Homelessness Improving Lives (P.H.I.L) - Ukraine

PHIL is an extension of Housing Options service for each LA within Warwickshire. Our Services cover those people who are registered on Homes For Ukraine Scheme Warwickshire, however those who are on Family Scheme, and Ukraine Extension Scheme can also seek general housing advice. The service ensures that all guests and sponsors are equipped with the resources and knowledge around them to explore future housing need to prevent a housing crisis situation from arising.

Hope 4

The Hope 4 charity helps homeless, badly housed and hungry people in Rugby. They give people food, warmth, laundry and showers, Winter Night Shelter from December through to the end of March and emergency food packages.

The CWPT Community Mental Health Team for Homeless People

Coventry and Warwickshire Partnership NHS Trust has employed a team of mental health professionals who offer a specialist clinical role for adults identified as Rough Sleepers, homeless or at risk of becoming homeless, who may have both unmet complex and severe mental health problems and associated social problems. Increased morbidity and mortality rates is a prominent issue among people who have experienced homelessness. To help reduce these health inequalities, this dedicated mental health team have been deployed to work within and alongside local voluntary and statutory organisations to support homeless individuals. The main premise of the service is to:

  • identify and reduce health inequalities,
  • improve health outcomes through person-centered care
  • strengthen the multi-disciplinary and multi-agency approach

The role is that of expert advice, liaison and therapeutic intervention; skills in trauma focused level one stabilisation work as well as specialist skills in CBA, CBT and Compassion focused therapy approaches; plus, motivational interviewing and solution focused skills are all available to be utilised. Referrals are accepted from Street Outreach Teams (SOT), local Hostels and Housing organisations, MDT meetings via rough sleeping initiatives/ housing tactical meetings, local authorities, CGL, police, probation, self-referrals through drop in provisions plus third sector plus Secondary care mental
health services.
